Terms and condition

Swift Samples provides mobile phlebotomy services to clients within our designated service area. Availability of appointments is subject to scheduling and location. Same-day or urgent appointments may be requested but are subject to availability.

It is the client's responsibility to ensure that accurate and complete information is provided at the time of booking, including but not limited to, personal details, medical history (if applicable), and insurance information (if relevant). Clients must also inform Swift Samples of any specific medical conditions, allergies, or special requests related to their service.

Payments for services rendered are due at the time of service unless otherwise agreed upon. Accepted methods of payment include credit/debit cards and other forms of payment as specified by Swift Samples. By providing payment, you authorize Swift Samples to charge the applicable amount for the services provided.

Clients may cancel or reschedule appointments with at least 24 hours’ notice to avoid a cancellation fee. Cancellations made less than 24 hours before the scheduled appointment may incur a fee. Swift Samples reserves the right to cancel or reschedule appointments at its discretion, with as much advance notice as possible to the client.

Swift Samples is a fee-for-service provider. Insurance reimbursement is the responsibility of the client. Clients should verify with their insurance company regarding coverage for mobile phlebotomy services. Receipts will be provided upon request for submission to your insurance provider.

Swift Samples adheres to strict privacy and confidentiality standards in compliance with HIPAA regulations. All personal and medical information provided by the client will be kept secure and confidential. Samples collected are handled with the utmost care, ensuring proper labeling and delivery to partner laboratories.

Clients agree to provide accurate information about their health conditions that may affect the blood draw process. Swift Samples reserves the right to decline service if it is deemed unsafe to proceed with the blood draw due to medical conditions or other factors. If any medical emergencies arise during the procedure, Swift Samples will take immediate steps to ensure the safety of the client, but the client may need to seek further medical care.

Swift Samples is not responsible for any errors or delays in laboratory results, nor for any consequences resulting from the failure to receive or act on test results. Our responsibility is limited to the collection and secure delivery of specimens to partner laboratories. By using our services, clients agree to release Swift Samples from any liability for delays, inaccuracies, or issues arising from laboratory processing or the client’s failure to seek follow-up care.

Swift Samples reserves the right to modify, suspend, or discontinue any part of the services offered, or alter the terms of these Terms and Conditions at any time. Clients will be notified of any changes that may affect existing bookings.

By booking a phlebotomy appointment with Swift Samples, clients provide consent for the collection, handling, and delivery of specimens for laboratory testing. The client acknowledges that they have been informed of the nature of the procedure and agree to the terms and conditions outlined.
